We’re honored to participate in the care of your client and grateful for your trust! Because we know these clients are already receiving high quality care and we are serving in a limited capacity in this arrangement, we are able to provide discounted consultation rates.
To schedule, direct your client to schedule a “new patient visit” and then indicate on the intake paperwork their referral source and specific needs. Your client will not be charged at the time of booking and their consultation fee will be adjusted to the rates below at the time of visit.
While RIH is not contracted with any insurance company, we can provide your clients with a superbill to submit themselves or to utilize HSA funds.
*Please note that while we always appreciate word of mouth referrals, we are only able to offer discounted pricing for single specific needs, rather than simply establishing ongoing care.
Discounted Rates by Referral
New Patient Consultation $200
Problem focused visit by referral for a specific need, including lab abnormalities, prescription management, ordering of specific testing or other concerns as needed. Revival will provide collaborative care for these clients and communicate all treatment recommendations to the referring provider, given patient consent to do so.
Follow up $185
Established clients needing follow up care.
