HOW IT WORKS

OPTION 1: NEW PATIENT INITIAL CONSULT

This is a 60-minute comprehensive appointment that includes a deep dive into your health history, current concerns, and full lifestyle assessment. If you have prior labs, you’re welcome to bring them to this visit. We’ll begin forming a personalized strategy and discuss next steps for testing, treatment and support.

This appointment is required for all new patients before ordering labs or starting a treatment plan.

OPTION 2: CONTINUOUS GLUCOSE MONITOR ONLY

If you’re curious about how your body handles food, stress, and sleep, a CGM can offer powerful insight. This 15-minute educational visit gets you set up with your monitor and ready to track your own glucose patterns.

If you find you need more support and you’d like a full comprehensive consultation within 30 days, we’ll apply 50% of this cost towards your second visit.

Already working with us? CGM setup is included with your package at no extra cost if requested.

Complete any labs ordered by your provider. This may include a blood draw at Quest Diagnostics, or a variety of specialty panels collected in the comfort of your home. No time to head to a lab appointment? No problem- ask us about options for at-home blood draw options.

After your initial consult, follow-up visits are scheduled as needed to review labs, adjust your care plan, and continue your progress.

BASIC FOLLOW UP

30 minutes to follow up on an existing plan, review conventional labs like Quest or Labcorp, or address a single new concern.

COMPLEX FOLLOW UP

45 minutes to review functional testing such as DUTCH, Nutreval, GI Effects, or discuss complex ongoing issues or multiple new concerns. Functional lab panels always require a Complex Follow Up for review.

Need more support between visits? Concierge Care gives you direct access to your provider via text and email for real-time help and fine-tuning.

Text us from the grocery store with a photo of a label. Reach out for guidance when you hit a wall in your protocol. This add-on is for patients who want an extra layer of support without waiting for the next visit.

Concierge care is for active patients only and not available as a stand-alone service.

Once you book your first appointment, you’ll receive intake forms to complete to get started. Please complete these a minimum of 24 hours prior to your visit, to allow for deep, careful review.
