BEGIN MEMBERSHIP

Starting your membership is simple. Choose your provider, and schedule a New Patient Visit. In the 'Reason for Appointment' box when scheduling, please indicate that you would like to begin membership. We'll send you the membership contract to review and sign. Your membership will begin the day of your first appointment, and your card on file charged at that time.
