Integrative + Functional Medicine, With More Support: Inside the Revival Membership

By Amanda Queen, PA-C

If you've ever left a medical appointment thinking, “I finally found someone who is listening, but now what?”, you're not alone.

For many people with complex or persistent health concerns, the challenge isn't simply finding a provider. It's having enough time, consistency, and access to actually work through the problems.

Functional and integrative medicine often requires looking at patterns over time. Symptoms change. Lab results come back in stages. Treatments need to be adjusted. Nutrition and lifestyle strategies may need to be modified as your body responds.

This is why we created the Revival Integrative Health Membership.

Membership isn't required to become a patient at Revival. We continue to offer traditional pay-per-visit care for patients who prefer that model. But for people who want more consistent support and a closer relationship with their provider, membership can offer a very different healthcare experience.

Why ongoing care matters in functional medicine

Functional medicine is rarely a “one appointment and you're done” process.

You may start with fatigue, digestive problems, hormonal changes, thyroid concerns, blood sugar issues, or several symptoms that don't seem connected. Understanding what's happening may require a detailed history, conventional laboratory testing, functional testing, nutrition and lifestyle assessment, and time to see how your body responds to an individualized plan.

Sometimes, the first plan isn't the final plan. A supplement may need to be adjusted. A lab result may change the direction of your care. A new symptom may emerge. You may make significant progress in one area while discovering another area that needs attention.

Good care needs room to evolve.

This is where consistent follow-up can make a meaningful difference.

Instead of seeing your provider every few months and trying to remember everything that has happened since your last appointment, membership allows your provider to stay closely involved in your care and make adjustments along the way.

What makes Revival membership different?

The Revival membership is designed around three things:

Regular visits. Concierge access. Comprehensive lab review.

1. You see your provider twice every month

Membership includes two provider visits every month.

Your first month begins with a comprehensive new-patient consultation followed by a dedicated lab review visit. From there, your twice-monthly visits can be used for follow-up, reviewing new laboratory results, addressing emerging concerns, and adjusting your plan.

That frequency matters because health doesn't always follow a convenient appointment schedule.

If something isn't working, you don't necessarily want to wait months to address it.

If you're making progress, you want to know how to build on it.

And if new information becomes available, you want someone who already knows your history to help you interpret it.

2. You have access between appointments

One of the most frustrating parts of conventional healthcare can be the gap between appointments.

You have a question about your plan. You aren't sure whether something you're experiencing is expected. You find a new product at the grocery store and want to know if it’s a good fit. You receive laboratory results and don't know what they mean.

With membership, you have extended direct access to your provider between visits. Instead of starting from scratch each time, you're communicating with someone who already understands your history and current treatment plan.

For many patients, this continuity is one of the most valuable parts of membership.

3. Your labs don't have to fit into a single appointment

Functional medicine can involve a significant amount of data.

You might have conventional blood work alongside a DUTCH hormone test, GI Effects stool testing, NutrEval, or another specialty panel.

With membership, laboratory review is included within your regular visits, without a limit on the number of results your provider can review. Testing itself is separate from the membership fee, but functional testing is available at provider pricing with no markup, and discounted cash-pay options are available through Quest.

Who tends to benefit most from membership?

Membership may be particularly helpful if you feel like your health requires more attention than a traditional appointment schedule allows.

You may be a good fit if:

  • You've been dealing with symptoms for a long time without a satisfying explanation.

  • You've been told your labs are “normal,” but you don't feel normal.

  • You have multiple overlapping concerns rather than one isolated issue.

  • You're working through gut health, hormone, thyroid, metabolic, or inflammatory concerns.

  • You've tried several approaches and need help determining what is actually working.

  • You want more accountability and support implementing changes.

  • You want a provider who knows your history rather than explaining everything from the beginning at every appointment.

  • You value having someone available when questions arise between visits.

The membership model can be especially useful when your health concerns are complex, evolving, or require frequent adjustments.

Membership isn't necessarily for everyone

We don't believe everyone needs concierge care.

If you have a straightforward concern, need occasional follow-up, or prefer to schedule appointments as needed, our standard pay-per-visit model may be a better fit.

Revival offers comprehensive new-patient consultations, basic follow-ups, and complex follow-ups for functional laboratory review and more involved concerns.

We also offer discounted visit packages for patients who want to prepay for four or more visits.

Membership is for patients who want something different: a more continuous relationship with their provider and a higher level of access.

What does Revival membership cost?

The membership is $500 per month with a 12-month commitment, but this introductory pricing is only available until September 30, 2026.

That includes:

  • Two provider visits every month

  • Unlimited direct messaging and email access

  • Unlimited laboratory review within your visits

  • Discounted cash-pay Quest Diagnostics laboratory testing

  • Functional laboratory testing at provider cost with no markup

  • 30% off professional-grade supplements through Fullscript, available to all Revival patients

The membership is intentionally structured as an annual commitment because meaningful health changes generally take time.

At the end of the first year, your needs may look very different than they did when you started. At that point, you can continue month-to-month or transition to less frequent, such as quarterly, follow-up depending on your health and goals.

The real value is continuity

We think the biggest benefit of membership isn't simply the number of appointments.

It's continuity.

Your provider sees where you started.

They know what you've tried.

They know which interventions helped and which didn't.

They can see your laboratory trends over time.

They understand the context behind your symptoms.

And when something changes, you don't have to begin the conversation all over again.

That continuity allows care to become more responsive and individualized.

Rather than asking, “What can we do about this symptom today?”, the conversation becomes:

“Where are you now, what has changed, and what is the next best step?”

That is the kind of relationship we believe healthcare should be.

Convenient care, wherever you are

Revival Integrative Health is a fully virtual practice. Visits are conducted via telehealth, and communication takes place through our secure patient portal, allowing you to receive ongoing care from home.

We serve patients in Arizona, California, Florida, Michigan, Missouri, North Carolina, Pennsylvania, Tennessee, Texas, and Virginia.

Ready for a different kind of healthcare experience?

You don't have to choose membership simply because you're interested in functional medicine.

But if you've been looking for a provider who has the time to understand the bigger picture, stays involved as your health evolves, and is available when questions arise between appointments, membership may be the right level of care for you.

Revival membership is intentionally limited so that we can maintain the time, attention, and accessibility that make this model work.
